To Our Referring Doctors,

We appreciate your confidence in our patient care.  Please complete the following steps to schedule your patient's appointment at our office.

1.  Visit our scheduling page to schedule your patient's appointment at our office.  After you have scheduled the appointment, your patient will receive an email/text message with the information for their appointment. 

2.  Kindly fill out our referral card and email it to our office along with the patient's most recent x-rays.
